and communicated to your doctor for his/her input and approval. |

| Your participation is essential to your healing. |
| Once a plan for healing your wound is developed, you
will need to follow that plan to the best of your ability to achieve the best and fastest
outcomes. |
| You are encouraged to call the Reeb Center at any
time with questions or concerns. |

| Preparing for your first visit
| Share the information from this web site with your
doctor. Ask your doctor to call the Reeb Center for Wound Management to order
services. If you prefer, the Reeb Center will contact your doctor to further
describe the program. |
| Call the Reeb Center at (847) 368-0918 to schedule an
appointment. Provide insurance information at this time so that coverage for the services
and supplies can be determined prior to your visit. |
| On the day of your visit, bring a list of your drug
allergies and medications, your insurance card, and do not change your dressing for at
least 8 hours. |
